ABSTRACT

New medical devices are developed continuously, so through assessment of needs, verification of safety and efficacy, rational procurement, proper installation, preventive maintenance, rational use and quality assurance, a better use of resources is accomplished.
Biomedical engineers play an important role in the proper design, development, selection, management, maintenance and user training of medical devices. Effective engineering management prevents a disproportionate rise in the maintenance costs, and improves the quality and safety of health-care delivery. The selection and use of medical devices has to meet priority needs, be in accordance with the existing infrastructure and services and have a budget for operation and maintenance, in order to prevent misdiagnosis or delays in treatment due to lack of available medical equipment.
This Malaysian Standard applies only to all active medical equipment placed for use in any healthcare facility which requires maintenance. It is also intended to help them set up systems that will minimize risks associated with the use of those medical devices. "Active medical device" means any medical device (medical equipment) relying for its functioning on a source of electrical energy or any source of power other than that directly generated by the human body or gravity.
According to WHO, it is estimated that around 50% of medical equipment in developing countries is not functioning, not used correctly, and invariably not maintained, with serious consequences for patient care. This may be because the equipment was not needed or not appropriate, and most often lies idle for want of a spare part. It is critical that a medical device management policy exists that includes a financial provision for maintenance, spare parts and training in the initial cost of the equipment.
This Code of Practice guideline includes; Responsibilities of Biomedical Engineering services, Biomedical Engineer/technician and Medical device user and on Maintenance；Schedule maintenance included planned preventive maintenance PPM, calibration, user maintenance, Unscheduled maintenance, Acceptance tests for newly delivered devices. Also on Management; Mechanisms to avoid failure or breakdown during use, Uptime, Quality assurance program (QAP), Maintenance management information system (MMIS), Management of warranties, Decommissioned equipment, Processes for handling hazardous/contaminated equipment, Incidents and hazards, User training, Stock of genuine spares parts, On site library, Workshop setup, Advisory service, Procurement of equipment.

 
アクティブな医療機器を工学的に良好に維持管理するための実施基準

新しい医療装置が次々と開発されるなかで、需要の評価、安全性と有効性の確認、合理的な調達、適切な設置、予防的保全、合理的な使用と品質保証を通して、より良い資源の活用が達成される。
生物医学エンジニアは、医療装置の適切な設計、開発、選択、管理、保全およびユーザのトレーニングで重要な役割を果たす。効果的な工学的管理は不釣り合いな維持費の増大を防ぎ、提供される医療の品質と安全性を改善する。利用可能な医療機器の不足に起因する誤診または処置の遅れを防ぐため、医療機器の選択と使用は、優先ニーズならびに既存のインフラやサービスと合致せねばならず、運用・管理のための予算がついていなければならない。
このマレーシアの規格は、保全を必要とするすべての医療機関に設置されるすべてのアクティブな医療機器だけに当てはまる。それは、それら医療機器の使用による危険を最小化するためのシステムを設置する際の援助をも企図している。「アクティブな医療機器」とは、人力または重力によって直接発生するのではない、電気エネルギー源またはそれ以外の動力源に依存するあらゆる医療機器を意味している。
WHOは、発展途上国のおよそ50%の医療機器が機能しておらず、正しく使われておらず、定常的に維持されておらず、患者の治療に深刻な結果をもたらしていると考えている。その理由は、機器が不必要あるいは不適切であるためであるかも知れず、また予備部品の不足による遊休化が非常にしばしば起きている。保全、予備部品とトレーニングに対する財政的な準備を機器の初期コストに含める医療機器管理方針の存在が非常に重要である。
実施基準ガイドラインには、次の項目を含んでいる。；生体工学的サービス、生物医学エンジニア／テクニシャン、医療機器使用者の責務。保全に関して；計画的な予防保全（PPM）を含む定期保全、較正、ユーザ保全、不定期保全、新規導入装置の受け入れ試験。管理に関して；使用中の不良や故障を避けるメカニズム、動作可能時間、品質保証プログラム（QAP）、保全管理情報システム（MMIS）、保証管理、廃棄機器、危険な／汚染された機器の取扱い作業、事故・障害、ユーザ・トレーニング、純正保守部品の在庫、オンサイト図書館、ワークショップの準備、顧問業務、機器の調達。
